Hanover Park Regional High School District -

To become a substitute teacher or nurse for the Hanover Park Regional High School School District at Hanover Park and Whippany Park High School, candidates must possess either a valid New Jersey Teaching Certificate or a valid Substitute Certificate. Following the submission of all required documents an interview may be scheduled.

• Tuberculosis Screening Record (within the previous six months).

• W-4 Form​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​

• Employment Eligibility Form – I-9​​​​​​​​​​​​​​​​​​​​​​​​​​​​ and required documentation

Please send the following information to our attention:

• Application

• Oath Form

• Tuberculosis Screening Record (within the previous six months).

• Harassment, Intimidation and Bullying Prevention course completion certificate.

• Copy of either your NJ Teaching Certificate or your Substitute Certificate

•  See attached Criminal History Record Check​​​​​​​ instructions.  The cost to be fingerprinted is $68.98.  IndentoGo will assign a PCN number and return the above form to you at your appointment with a receipt attached. An $11.00 administrative fee will be charged prior to scheduling an appointment to be fingerprinted.
  

• If your fingerprints were processed by another NJ public school after February 2003, the State will look in their archives to reprocess your information.  Complete the information at the State Criminal History Review Unit website:

https://www.nj.gov/education/crimhist/ for Archive Application Request​​​​​​​ and provide our office with a copy of the receipt issued.  The cost for archive retrieval is $30.55, which includes the $11.00 administrative fee ($1.00 of which goes to NICUSA).

Substitute School Nurse:

In addition to the items listed above for a Substitute Nurse Certificate, please provide a copy of a valid New Jersey registered professional nurse license (photo copy acceptable).
